WHEREAS, this council is cognizant of the
increasing number of public and private owners and operators of vehicles that
ply over Twenty Four Hours (24) a day in Bauan;
WHEREAS, this increasing number should be
considered by LTO and provide them with service that is very accessible and
comfortable to them;
WHEREAS, it is only appropriate that
considering the human and vehicular traffic that congest Bauan Twenty Four (24)
Hours a day the LTO shall innovate the creation of a satellite office in Bauan
to make its service closer to a great number of owners and operators of
vehicles that reside in Bauan and nearby towns;
NOW THEREFORE, CONSIDERING THE
FOREGOING, BE IT RESOLVED, as it is hereby resolved by this august body to request the
LTO to innovate the creation and putting up of a satellite office in Poblacion
Area of Bauan, this municipality.

REQUESTING THE PHILHEALTH OFFICE TO
ESTABLISH
A BRANCH IN BAUAN
(SPONSOR: HON. JULIAN C. CASAPAO)
WHEREAS, this council finds it
necessary to put up a PHILHEALTH office in the Municipality of Bauan;
WHEREAS, this office will help
our constituents to avail of a much faster way delivery of services;
WHEREAS, this request is not only
for the people of Bauan but for other municipalities;
NOW, THEREFORE, BE IT RESOLVED as it is hereby resolved by the
Members of Sangguniang Bayan to pass and approved this resolution.

RESOLUTION
NO. 12-02-0454
APPROVING THE CREATION OF AN
INFORMATION TECHNOLOGY (IT) OFFICE THAT WILL ABSORB THE PERSONNEL CURRENTLY
DETAILED AT THE MIS TO ENSURE THE CONTINUITY OF THE OPERATION OF
COMPUTERIZATION SYSTEM, SO AS NOT TO LEAD IN THE WASTAGE OF MUNICIPAL FUND,
THIS MUNICIPALITY

and HON. HERMINIO M. DOLOR)
WHEREAS, Mr. Antonio V. De Lacy Jr.,
Municipal Administrator, System Administrator, CeC Manager, sent a letter
request to Hon. Juan M. Magboo, Vice Mayor and SB Presiding Officer dated
August 26, 2011 about draft resolution no. 11-09-414;
WHEREAS, the Management and Information
System (MIS) is a component office of the Local Government of Bauan functioning
since 2008;
WHEREAS, it was primarily tasked to maintain
the integrity and continuity of the Local Government’s Automated Revenue
Collection System;
WHEREAS, the office was also in-charge of
maintaining and conditioning all the twenty three (23) workstations of the
Revenue-Generating Units within the offices of the Municipal Assessor;
WHEREAS, owing to the non-existence of an
Electronic Data Processing (EDP) Office to cater to EDP-Related Services of all
non-LAN computer units installed and used by different offices in the Municipal
Building, MIS Office also took charge of providing these equally-important
services;
WHEREAS, with the computerization expansion
made for the BPLS, the COA had sent an Audit Observation Memorandum (AOM) numbered
2011-16 (2010) dated February 28, 2011 recommending the creation of an
Information Technology (IT) Office that will absorb the personnel currently
detailed at the MIS Office to ensure the continuity of the operation of
computerization system, so as not to lead in the wastage of Municipal fund;
WHEREAS, available Plantilla positions had
been cleared with the Personnel Office creation of the ICT Department as
follows:
- Municipal Government Department Head
- Audio-Visual Technician I
- Electronics and Communications Equipment Technician
- Computer Programmer I
WHEREAS, relative thereto the undersigned
formally request for the passage/approval of a resolution regarding this
matter.
NOW, THEREFORE, BE IT RESOLVED AS IT
IS HEREBY RESOLVED
that Draft Resolution No. 11-09-414, Approving the creation of an Information
Technology (IT) Office that will absorb the personnel currently detailed at the
MIS Office ensure the continuity of the operation of computerization system, so
as not to lead in the wastage of Municipal fund be approved by the Sangguniang
Bayan Members, this municipality.

TO A JURIDICAL ENTITY LICENSE IN THE NAME OF THE LOCAL
GOVERNMENT UNIT OF BAUAN, BATANGAS

WHEREAS, Hon. Juan M. Magboo, SB
Presiding Officer of Sangguniang Bayan thru Mr. Ferdinand M. Agena received a
letter from Hon. Mayor Ryanh M. Dolor dated January 18, 2012;
WHEREAS, the local government of Bauan
has in its inventory firearms issued to municipal employees/officials;
WHEREAS, the local government of Bauan
has applications for the issuance of Provisional License to legally posses
issued in the name of JOSE S. DE MESA, Property Custodian of the Municipality of Bauan, Batangas;
WHEREAS, the transfer from individual
firearm licenses to a juridical entity license will improve the accounting of
such firearms;
NOW, THEREFORE, BE IT RESOLVED AS IT IS
HEREBY RESOLVED by the Hon. Members of the Sangguniang Bayan that the Draft
Resolution No. 12-02-473, requesting the Philippine National Police (PNP) Camp
Crame, Quezon City to transfer from -individual firearm licenses to a juridical
entity license in the name of the local government unit of Bauan, Batangas be
approved.
